A non-ergoline Dopamine Agonist, Anti-Parkinson`s Agent.
Mechanism of Action of Ropinirole
Ropinirole is a non-ergoline Dopamine agonist. The antiparkinsonian activity is due to stimulatory effect on post synaptic Dopamine receptors (D2) within the caudate- putamen. It has affinity for other dopamine receptors such as D3 &D4. Ropinirole also has moderate invitro affinity for opioid receptors. Ropinirole and its metabolite has very little affinity for D1, 5HT1, 5HT2,Benzodiazepines, GABA, muscarinic, alpha and beta adreno receptors.
Pharmacokinets of Ropinirole
Absorption: Ropinirole is rapidly absorbed after oral administration. Its bioavailability is about 55% only and this indicates that it undergoes first pass metabolism.
Distribution: It is very widely distributed in the body.
Metabolism: It is extensively metabolised in the liver to inactive metabolites. Excretion: Ropinirole is excreted mainly in the urine.

Indications for Ropinirole
1. Parkinson`s disease - idiopathic Parkinson`s disease.
Interactions for Ropinirole
Oestrogens decrease the oral clearance of ropinirole thereby increasing its plasma levels.
Cimetidine, ciprofloxacin, Diltiazem and Mexiletene increase its levels by altering the clearance.
Phenothiazines and butyrophenones decrease its effectiveness.
Typical Dosage for Ropinirole
Adult: 0.75 mg / day in 3 divided doses. Depending on patient`s response, dose can be gradually increased by 0.75 mg / day every week. After 4 week dose is increased by 1 - 1.5 mg / day up to 9 mg / day. Dose can be again increased by 3 mg / day every week.
Maximum dose: 24 mg / day

Store at 20 - 25 degree C in a tightly closed container. Protect from heat and moisture. Keep out of the reach of children.
Effects of Missed Dosage of Ropinirole
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Ropinirole
Give supportive measures and symptomatic treatment. Drug can be removed from the body by gastric lavage.
