Dopamine Agonist, Ergot- Alkaloid, Anti Parkinsonsm, hypoprolactinemic.
Mechanism of Action of Bromocriptine
Bromocriptine is a dopaminergic agonist. It acts as a potent agonist on D2 receptor and partial agonist or antagonist on D1 receptor. Bromocriptine produces a decrease in Dopamine turnover with increase in Dopamine content of the brain and decreases the metabolism of Dopamine. The increase in Dopamine concentration produces effect in Parkinson`s disease.
Since Dopamine is a neurotransmitter in the brain it can produces various motor, behavioural and endocrine effects.
Hormonal action: Bromocriptine inhibits synthesis and release of Prolactin from pituitary lactotrope cell by acting as a D2 agonist.
Bromocriptine increases growth hormone release in normal individual and decreases the growth hormone in patient with acromegaly.
Bromocriptine produces nausea and vomiting by stimulating dopaminergic receptors in chemoreceptor trigger zone (CTZ)
Pharmacokinets of Bromocriptine
Absorption: Only 1/3 of Bromocriptine is absorbed after oral administration since it undergoes first pass metabolism. Distribution: It is distributed in protein bound form. Metabolism: It is completely metabolised in the liver. Excretion: It is excreted primarily in faeces. Only a very small amount excreted in urine.

Interactions for Bromocriptine
Antihypertensives: Efficacy potentiated by concomitant use of bromocriptine dopamine antagonists.
Phenothiazines, butyrophenones: Decreases efficacy of these drugs.
Erythromycin: increases efficacy of bromocriptine.
Alcohol: Decreases tolerance to bromocriptine and vice-versa.
Typical Dosage for Bromocriptine
Adult:
Parkinson`s disease:
Initial dose: 1 - 1.25 mg at night with food for 1 week.
2 - 2.5 mg at night with food for 2nd week
5 mg / day in 2 divided doses for 3rd week
7.5 mg / day in 3 divided doses for 4th week.Then increase the dose by 2.5 mg daily every 3 - 14 days until sufficient therapeutic response is obtained
Maintenance dose: 10 - 40 mg/day.Maximum dose: 100 mg / day
Amenorrhea and Galactorrhea associated with hyperprolactinemia: Initial dose: 1.25 - 2.5 mg / day increase the dose by 2.5 mg daily every 3 - 7 days until sufficient therapeutic response is obtained.
Maintenance dose: 5 - 7.5 mg/day.Maximum dose: 40 mg / day
Acromegaly: Initial dose: 1.25 - 2.5 mg at night for 3 days increase the dose by 1.25 - 2.5 mg daily every 3 - 7 days until sufficient therapeutic response is obtained.Dose range: 20 - 30 mg / day in divided doses. Maximum dose: 100 mg / day
Hepatic encephalopathy: Initial dose: 1.25 mg / day increase the dose by 1.25 mg every 3 days until sufficient therapeutic response (15 mg) is obtained.
Premenstrual syndrome 5 -15 mg in 2 divided doses 10th day of menstrual cycle until onset of menstruation
Cushing`s syndrome: 1.25 - 2.5 mg 2 - 4 times daily
Suppression of established lactation: 2.5mg daily for 2 to 3 days. Subsequently increased to 2.5mg twice daily for two weeks.
Children: Not recommended
