Diphenylbutylpiperidine derivative, Antipsychotic, Antidyskinetic.
Mechanism of Action of Pimozide
Pimozide exerts its antipsychotic action by selectively inhibiting the Dopamine receptor. It also possesses weak antiadrenergic, antihistaminic, antiserotonin and anticholinergic actions. It also inhibits the reuptake of dopamine from the synaptic cleft

Absorption: Pimozide is absorbed slowly after oral administration and its bioavailability is only 50%.
Distribution: It is distributed widely in the body.
Metabolism: Pimozide extensively undergoes hepatic metabolism.
Excretion: Excreted mainly in urine and also in faeces.

1. Chronic psychoses
2. Gilles de la Tourette syndrome
Interactions for Pimozide
Anticonvulsants: Pimozide may lower the convulsive threshold.
Phenothiazines, TCAs or Antiarrhythmic agents: Pimozide prolongs the QT interval, therefore avoid concomitant use.
Alcohol and other CNS depressants (Hypnotics, Sedatives, Analgesics, Anxiolytics etc.): Pimozide potentiates the effects of these drugs.
Typical Dosage for Pimozide
Adults:
Chronic psychoses:2 mg / day single dose at night or in divided doses dose can be gradually increased by 2 - 4 mg every weak if necessary.Maintenance dose: 6 mg / day.Maximum dose: 20 mg / day
Gilles de la Tourette syndrome: 1 - 2mg / day in divided doses; as needed dose can be gradually increased.
Maximum dose: 10 mg / day

Storage Requirements for Pimozide
Store at 15 - 30 degree C. Keep out of the reach of children.
Effects of Missed Dosage of Pimozide
Take the missed dose as soon as noticed. If it is the time to take the next dose then skip the missed dose and take that dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Pimozide
Give supportive measures and symptomatic treatment. To remove the drug from the body gastric lavage can be done. Maintain blood pressure with IV fluids and Extrapyramidal symptom with Diphenhydramine.
