Beta Blocker With Alpha- Adrenergic Blocking Activity, Antihypertensive and Anti angina.
Mechanism of Action of Carvedilol
Carvedilol is an adrenergic antagonist capable of blocking alpha-1, beta-1, and beta-2 receptors. It produces vasodilatation through alpha-1 blockade as well as direct action. It decreases peripheral resistance, heart rate and blood pressure. It prevents reflex tachycardia. It also exerts it`s pharmacological actions through additional actions like Ca2+ entry blockade , anti oxidant action, antiproliferative action, & membrane stabilizing action. It does not harm lipid profile.Carvedilol improves ventricular function and reduces morbidity and mortality in patients with congestive heart failure.

Absorption: Well absorbed orally, but first pass metabolism reduces extend of bioavailability. Distribution: Extensively distributed in to extra vascular tissues in a protein bound form.
Metabolism: Metabolized in liver. Excretion: Excreted mainly through bile and faeces. A small portion is excreted through urine

Indications for Carvedilol
1.Hypertension
2.Congestive heart failure
3.Stable angina pectoris
Interactions for Carvedilol
Antidiabetic agents : Effects of insulin and oral hypoglycaemics enhanced.
Calcium blockers : Isolated cases of conduction disturbance have been observed.
Clonidine: Blood pressure and heart rate lowering effects potentiated.
Digoxin : Serum concentrations of digoxin are increased requiring monitoring of digoxin.
Rifampicin : Plasma concentration of carvedilol reduced by 70%.
Typical Dosage for Carvedilol
Initial dosage: 6.25mg to 12.5mg once daily for 2days. Increases the dosage by 12.5mg up to 50mg once daily based on patient`s response.
Congestive heart failure: Starts with 3.125mg and gradually increases to 25mg 12th hourly.
Angina: Starts with 12.5mg 12th hourly and gradually increases to 25-50mg 12th hourly

Effects of Missed Dosage of Carvedilol
Take the missed dose as soon as noticed and if it is the time to take the next dose then skip the missed dose.
Effects of Overdose of Carvedilol
Treatment is supportive and symptomatic. After ingestion empty the stomach by induced emesis or gastric lavage. Administer activated charcoal to reduce absorption. Keep patient in supine position and monitor and correct vital parameters. Treat Bradycardia with Atropine. If required administer Isoproterinol with caution; in heart block. Treat hypotension with Glucagons and vasopressors like Epinephrine. Treat cardiac failure with diuretics and cardiac glycosides. Treat seizures with I.V. Diazepam.
