Mechanism of Action of Famciclovir

Famciclovir is a prodrug and it is converted to active Penciclovir. It undergoes phosphorylation in the presence Thymidine kinase and form Penciclovir monophosphate and it is again phosphorylated by Cellular kinase and form Penciclovir triphosphate. This Penciclovir triphosphate inhibits DNA polymerase enzyme and inhibits DNA synthesis and consequently inhibit viral replication.

Uricosurics: probenicid possibly reduces famciclovir excretion thereby increasing plasma concentration.

Herpes zoster infection in immunocompromised patient: 1.5 g / day in 3divided dose for 1 week
Prophylaxis of herpes simplex virus in immunocompromised patient: 1 g / day in 2 divided dose
Genital herpes in immunocompromised patient: 250 mg / day in 2 divided dose for 5 days
