Aminoglycoside Antibiotic, Antitubercular Agent.
Mechanism of Action of Streptomycin
Streptomycin exerts it`s bactericidal action against gram- negative organisms & some of gram positive organisms by inhibiting bacterial protein synthesis. The process involves 1. Penetration of bacterial cell membrane & binding directly to the 30 S subunit of bacterial ribosome, 2. misleading of m-RNA codon by bacterial ribosome, 3. Formation of peptide chains with wrong amino acid sequence which gets incorporated into bacterial cell membrane and alters it`s permeability, 4. Bacterial cell lysis.

Absorption: Poorly absorbed orally, but well absorbed intra muscularly., Distribution: Distributed extracellularly, Metabolism: Not metabolized, Excretion: Excreted through urine.

1. Damage to 8th cranial nerve & Ototoxicity

5. Super infection with Candida & Staphylococcus aureus
6. Some degree of nephrotoxicity


Contra-indications of Streptomycin
1.Hypersensitivity to amino glycosides
2.Labyrinthine disorders
3.Never administer I.V
4.Never use along with tetracycline & chloramphenicol
5.never use along with other nephrotoxic drugs

1.Tuberculosis
2.Brucellosis
3.Tularemia
4.Plague
5.Enterococcal endocarditis
Interactions for Streptomycin
Aminoglycosides, Amphotericin B, Bacitracin, Cisplatin, Cephalothin, Vancomycin, Methoxyflurane Ethacrynic Acid, Frusemide, Bumetanide & Mannitol: Increased ototoxic, neurotoxic or nephrotoxic affects.
Anaesthetics, Neuromuscular blocking agents (Tubocurarine, Gallamine): Risk of neuromuscular blockade and respiratory paralysis.
Penicillin, Cephalosporins, Carbenicillin, Ticarcillin: Exert antibiotic synergism, especially useful in Pseudomonas infections.
Lab tests: Aminoglycoside serum levels: Guard against in-vitro inactivation of aminoglycosides by b-lactum antibiotics in patients on combination therapy.
Typical Dosage for Streptomycin
0.5 to 1gm daily; Given intramuscularly.
Tuberculosis:
Adults: 15 mg/kg /day for three months then continue the therapy with same dose twice or thrice in a week
Children: 20 to 30 mg /kg/day in divided doses.
Brucellosis:
1gm daily for 21 days along with tetracycline.
Tularemia:
1 to 2gm IM for a week or more.
Plague:
30mg/kg/day twice daily for 10 days. For better results streptomycin is combined with tetracycline or chloramphenicol
Enterococcal infections:
1gm twice daily for two weeks; then 500mg twice daily for four weeks

Effects of Missed Dosage of Streptomycin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Streptomycin
Symptoms include allergic manifestations, Ototoxicity, Nephrotoxicity, Neuromuscular toxicity. Treatment include Dialysis, Administration of Prednisolone, Ca salts and Anticholinesterase like neostigmine.
