About Sucralfate
Pepsin inhibitor, Anti-secretory, Gastrointestinal Agent, antiulcer.
Mechanism of Action of Sucralfate
Sucralfate is a complex of sucrose, sulfate and aluminium hydroxide. In the stomach (pH<4) Sucralfate undergoes polymerization and form viscous sticky polymers that adhere to the surface of ulcers and coat the ulcer.
It also protects the ulcer surface from further injury by inhibiting the enzyme pepsin which hydrolysis mucosal proteins. Sucralfate increases the production of Prostaglandins and protect the lining of the stomach. It also binds to the epithelial growth factor and Fibroblast factor and repair the mechanism of stomach lining.

Antibacterials (ciprofloxacin, norfloxacin, ofloxacin and tetracycline), Antiepileptics (phenytoin), Antifungals (ketoconazole), Cardiac glycosides (digoxin), Thyroxine: Sucralfate may reduce the absorption of these drugs.
Anticoagulants: Reduced efficacy of warfarin.
Antacids (aluminium containing) :Total body burden of aluminium may be increased.
Cimetidine, Ranitidine and Theophylline: Reduced bioavailability of these drugs.
Quinidine: Serum quinidine levels may be reduced.
Typical Dosage for Sucralfate
Adult:
Oral dose: 4 g / day in 2 - 4 divided doses drug to be taken 1 hour before meals and at bed time.
Suspension: 10 ml to be taken 4 times daily.
Children: not recommended

Amoebiasis:
Adults: 2gm once daily for 3 days
Children: 30 to 50mg/kg/day or 600mg 12hourly for 5 to 10 days
Trichomoniasis & Giardiasis:
600mg once daily for a week or 2gm single dose
Anaerobic bacterial infections:
2 gm followed by 500mg 12 hourly for 5 days
Prophylaxis after colorectal surgery:
2gm single dose before surgery
H. pylori infections:
0.5gm 12hourly for 7 to14 days in triple combination

Mechanism of Action of Povidone Iodine
Povidone iodine is an iodophore antiseptic and is a microbicidal drug. It releases iodine from its complex and produces pharmacological actions. The released iodine produces antiseptic actions. Iodine is highly reactive with surrounding and has strong oxidizing effect on functional groups of amino acids (-NH 2, -SH etc.), Nucleotides, and fatty acids (double bond of unsaturated fatty acids). Topical
Adult:
Ointment (5% w/w Povidone iodine), Cream (5% w/w Povidone iodine). Spray(5 - 10 % w / v Povidone iodine solution), Solution (5 - 7.5 % w/ v Povidone iodine solution) and Powder( 5% w/w Povidone iodine)
Thoroughly clean and dry the area. Apply the drug. Cover with a bandage.
Gargle solution:
Oral candidiasis: 1% mouthwash solution rinse mouth with this solution
Oral hygiene: 0.1 % of 10 ml solution (undiluted or diluted with equal quantity of warm water solution) rinse mouth with this for 30 seconds 4 times daily for 2 weeks
Surgical scrub: 7.5 % w/v solution
Wet hands with water. Pour about 1 teaspoonful of Scrub on the palm of the hand and spread over both hands and
scrub thoroughly for about 5 minutes.
Use a brush if desired. Clean thoroughly under fingernails.
Add a little water and develop copious suds. Rinse thoroughly under running water.
Repeat the entire procedure.

Pessaries:
Vaginal candidiasis: 200 mg as pessaries. Insert 2 pessaries in vagina at night and continue for 2 weeks
