Distribution: Protein binding: 74%.
Metabolism: Hepatic via CYP2D6 and 3A4.
Excretion:. Excreted in urine (80%) and faeces (11%).
2.Extrapyramidal disorder, including involuntary movements, tremors and muscle stiffness

2.Avoid use in patients with Congenital long-QT syndrome or history of cardiac arrhythmia.

4.Increased risk of tardive dyskinesia in the elderly, especially women.
5.Monitor glucose control in patients with or with risk factors for diabetes or other disorders of glucose regulation.
6.Monitor for mental status changes, fever, muscle rigidity and/or autonomic instability.
7.May cause Orthostatic hypotension, impaired core body temperature, significant wt gain and/or sedation.
8.Monitor for behaviour changes or suicidal ideation.
9.Patients with Breast cancer or other prolactin-dependent tumors, Renal impairment or at risk of seizures.
10.May mask toxicity of other drugs or conditions e.g. Intestinal obstruction, Reye's syndrome, brain tumor.
11.Safety and efficacy have not been established in children <18 yr.

3.Serum levels may be reduced by carbamazepine.
4.Concurrent use may increase toxicity of other CNS depressants.

6.Lithium may increase neurotoxic effects of paliperidone.
