About MemantineN-Methyl-D-Aspartate Receptor Antagonist, dopamine D2 agonist, Antidementia agent,In Alzheimer`s disease.Mechanism of Action of MemantineMemantine in an uncompetitive (low to moderate affinity to) NMDA (N-methyl-D-aspartate) receptor antagonist. It regulates the activity of glutamate which is involved in information processing, storage and retrieval. Excess glutamate plays a role in neuronal cell death and it is common to all neurodegenerative diseases.
In Alzheimer`s disease excess release of glutamate occurs. Sustained elevation of glutamate over stimulates NMDA receptors and allows too much of calcium in to nerve cells and leads to cell degeneration and cell death. Memantine prevents these actions by inhibiting the binding of glutamate at NMDA receptor.

About DonepezilCentral Acetylcholinesterase inhibitor, Antidementia agent in Alzheimer`s disease.Mechanism of Action of DonepezilAlzheimer`s disease may result from a deficiency in neurotransmitters used by nerves in the brain to communicate with one another. Donepezil exerts its therapeutic effect by enhancing its cholinergic function. It increases the concentration of Acetylcholine through reversible inhibition of acetyl cholinesterase enzyme which is involved in the hydrolysis of Acetylcholine. It leads to increased concentration of Acetylcholine. This increased Acetylcholine concentrations are believed to be responsible for the improvement seen during treatment with donepezil.
Pharmacokinets of DonepezilAbsorption: It is well absorbed after oral administration. Distribution: Donepezil is widely distributed in the body in protein bound form. Metabolism: It is extensively metabolised in the liver. Excretion: Donepezil and its metabolites are excreted mainly in urine and small amount in faeces.Onset of Action for Donepezil2- 4 weeksDuration of Action for DonepezilN/AHalf Life of Donepezil70 hoursSide Effects of Donepezil1.Nausea

NEONATES: contraindicatedIndications for DonepezilAlzheimer`s disease (To improve memory, awareness, and the ability to function in patients)Interactions for DonepezilAntiarrhythmics: Procainamide, quinidine and probably propafenone antagonise effect of neostigmine & pyridostigmine.
Antibacterials : Aminoglycosides, clindamycin & colistin antagonise effect of neostigmine & pyridostigmine.
Antimuscarinics: Antagonism of effect.
Antimalarials: Chloroquine & hydroxychloroquine have potential to increase symptoms of myasthenia gravis & thus diminish effect of neostigmine & pyridostigmine.
Lithium: antagonism of effect of neostigmine & pyridostigmine.
Muscle relaxants: Possibly donepezil enhances effect of suxamethonium but antagonises effect of non-depolarising muscle relaxants. Typical Dosage for DonepezilAdult: 5 mg / day at bed time if needed after 4 weeks dose can be increased to 10 mg / daySchedule of DonepezilN/AStorage Requirements for DonepezilStore at room temperature. Protect from heat and light. Keep out of the reach of children.Effects of Missed Dosage of DonepezilIf you miss a dose skip the missed dose. Continue the regular schedule.
