About Thiopentone
An ultra-short-acting barbiturate(thiobarbiturate), Intravenous General anesthetic.
Mechanism of Action of Thiopentone
Thiopentone mainly acts on Picrotoxin site of GABA -BZD receptor chloride ion channel complex. It prolongs the duration of GABA induced chloride channel opening and produces hyperpolarisation and decreases firing rate of neurons. Thiopentone also has GABA mimetic action. Thiopentone will decreases presynaptic and postsynaptic excitation. Thiopentone depresses polysynaptic midbrain reticular activating system and produces anaesthesia.
Pharmacokinets of Thiopentone
Absorption: It is well absorbed after intravenous administration.
Distribution: It is initially distributed to highly vascular tissues in the body in protein bound form and then redistributed to fatty tissues.
Metabolism: It is extensively but slowly metabolised in the liver.
Excretion: Only a very small amount of the drug is excreted through urine.

Indications for Thiopentone
1. Anaesthesia
2. Rapid control of convulsions
Interactions for Thiopentone
Narcotics: Those of phenobarbitone required to induce anaesthesia reduced.
Phenothiazines: Increased frequency and severity of neuromuscular excitation and hypotension.
Typical Dosage for Thiopentone
Adult: Dosage should be individualised.
General anaesthesia: 3 - 5 mg / kg as 2.5 % solution
Control of convulsion: 50 - 125 mg as 2.5 % solution

Storage Requirements for Thiopentone
Store the dry form of powder at room temperature. The reconstituted solution is stable for 3 days at room temperature and for 1week if refrigerated
Effects of Missed Dosage of Thiopentone
Take the missed dose as soon s noticed and if it is the time to take the next dose then skip the missed dose.Do not double the dose.
Effects of Overdose of Thiopentone
Give supportive measures and symptomatic treatment. To treat hypotension IV fluids or vasopressors should be given.
