Mechanism of Action of Oxaliplatin
Oxaliplatin is a diaminocyclohexane platinum derivative with a broad spectrum of activity. It undergoes rapid non-enzymatic activation with the displacement of the oxalate ring by two chlorines.Chloride is replaced by water and form a positively charged molecule. Oxaliplatin is activated to a bis-aquated species through a number of reactions. The bis-aquated species then reacts with neighbouring guanine residues either on the same or neighbouring strands of DNA. This leads to the cell death. Its cytotoxic effects are more prominent at S phase but it kills the cells at all stages.

Absorption- Rapidly absorbed after intravenous administration Distribution- Widely distributed in the body. Platinum metal in the drug after degradation from it extensively and irreversibly bound to the plasma proteins.
Excretion- Excreted through urine as metabolites (small portion) and as unchanged form (large portion).

Initial dose : 85 mg/m2 IV infusion in 250-500 mL 5% Dextrose for 2 hours along with 5 Fluorouracil and Leucovorin.

Store under refrigerator (20-80 C) and protect from light.
Effects of Missed Dosage of Oxaliplatin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.

Give supportive and symptomatic treatment. Supportive treatment includes transfusion of blood components, antibiotics and antiemetics.
