A broad-spectrum antifungal antibiotic, imidazole derivative, Oral Nonabsorbed Antifungal Agent.
Mechanism of Action of Clotrimazole
Clotrimazole is fungicidal or fungistatic depending on the drug concentrations. It inhibits the conversion of Lanosterol to 14 demethyl Lanosterol by inhibiting the cytochromeP450 enzyme 14 alpha demethylase and impair ergosterol synthesis which is an essential constituent of cell membrane. Inhibition of ergosterol synthesis will alter the membrane permeability of the susceptible fungus and kill or inhibit them.

Polyene Antibiotics : Antagonises effects of polyene antibiotics.
Typical Dosage for Clotrimazole
Topical: Clotrimazole cream, lotion and solution 1 % is used.
Clean and dry the affected area and apply the medicament with gentle massage 2 -3 times daily for 2 - 4 weeks.
Vaginal candidiasis: Insert 1 applicator full (vaginal cream 1%) intravaginally at night for 1 -2 weeks.
Vaginal tablets 100. 200 and 500mg tablet is available.
Vulvovaginal candidiasis: Insert 100mg tablet intravaginally at bed time for 7 consecutive days or 200 mg tablet for 3 consecutive days or 500 mg tablet for 1 day at night.
Oral lozenge 10 mg is available
Oropharyngeal candidiasis: 1 lozenge orally 5 times daily for 2 weeks
Prophylaxis of Oropharyngeal candidiasis: 1 lozenge is used 3 times daily
Gel: Insert with an applicator intravaginally at night for 1 - 2 weeks.
Powder (1% w/ w): dusted over the body parts as required

A bisbiguanide antimicrobial agent, Anti-infective, antiseptic.
Mechanism of Action of Chlorhexidine
This drug has positive charge so it attach to the negative charge cell wall. This destabilizes and destroys the integrity and penetrates bacterial cell membranes. It results precipitation of the cytoplasm and interferes with membrane function by inhibiting oxygen utilization leading to a decrease in cellular ATP levels and cell death. In gram-negative bacteria, chlorhexidine affects the outer membrane allowing the release of periplasmic enzymes. The inner membrane of these organisms is not ruptured but the uptake of small molecules is impaired. At low concentrations chlorhexidine exhibits a bacteriostatic effect while at high concentrations it is bactericidal.

Typical Dosage for Chlorhexidine
Oral rinse solutions - 0.1 percent.
Otic (ear) solutions - 1 percent.
Chlorhexidine solutions - 2 percent.
Chlorhexidine shampoo and conditioner 1 and 2 percent.
As an ear-cleansing agent - flush the ear canal once a day in cases of diagnosed otitis externa.
For skin conditions - clean the area twice daily.
For gingivitis - Chlorhexidine oral solution is applied as directed once a day. In the morning brush and floss for 2 minutes and than rinse with Chlorhexidine for 30 seconds.
Use 1/2 fl. oz. of undiluted. Swish in mouth for 30 seconds. After rinsing for 30 seconds be sure to spit out all the rinse, do NOT swallow any of the rinse. To minimize medicinal taste, DO NOT rinse with water immediately after use. In the evening brush and floss and rinse again for 30 seconds.
Chlorhexidine shampoo and conditioner - once or twice a week.
Pellet (extended-release), Periochip (in a biodegradable matrix of hydrolyzed gelatin cross-linked with glutaraldehyde), Cream, Dressing, Gel, Lotion, Ointment, Sponge forms are also available.
