Ophthalmic NSAID, Indole derivative, Anti-inflammatory and antiallergic eye preparations.
Mechanism of Action of Indomethacin (Eye Prep.)
Indomethacin acts by inhibition of Prostaglandin (PGs) synthesis and their release at the site of injury. Prostaglandins cause tenderness and amplify the action of other algesics. Indomethacin does not affect the tenderness induced by PGs but block the pain producing mechanism induced by inflammatory mediators by inhibiting the enzyme cyclo-oxygenase. It has prominent analgesic, anti-inflammatory and antipyretic properties. It is more potent than Aspirin.
Indomethacin inhibits the motility of polymorphonuclear leukocytes and decreases the mucopolysaccharide biosynthesis. It also produces a cyclo-oxygenase independent vasoconstrictor effect.

Typical Dosage for Indomethacin (Eye Prep.)
Indomethacin 1%w/v or 0.5 % drops is used. 1-2 drops is instilled in to the eye for 6 - 8 hours daily.
Cystoid macular edema: Instill 1 drop 4 times daily for 2 - 3 weeks
Prevention of surgically induced miosis: Instill 1 drop 4 times a day, 24 hours before surgery and an additional dose of 1 drop is instilled 45 minutes before surgery.
To treat photophobia (sensitivity to light) occur after incisional refractive surgery: Start the medicine before the operation with 1 drop in the eye followed by same dose within one hour of surgery, 15 minutes after surgery, then 1 drop 4 times a day 4 -6 hours after surgery and continuing for 3 days as needed.
To relieve inflammation following cataract surgery: Start from 24 hours after cataract surgery instill 1 drop in the eye 4 times a day for 2 weeks.

Store at cool dry place. Protect from heat and direct light. Do not freeze. Keep out of the reach of children. Avoid use of medicament 1 month after opening.
Effects of Missed Dosage of Indomethacin (Eye Prep.)
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
