About LoperamideA synthetic piperidine derivative, Opioid, Antidiarrhoeal, Analgesic.Mechanism of Action of LoperamideLoperamide is a mu receptor agonist. It inhibits the release of Acetylcholine on myenteric plexus in the gastrointestinal tract. This results in slowing intestinal motility and reduces peristalsis and increase intestinal transit time. This will results in increased absorption of water from fecal matter. Loperamide also has a direct action on intestinal smooth muscle. It does not have any effect on central nervous system.Pharmacokinets of LoperamideAbsorption: Loperamide is poorly absorbed after oral administration

Metabolism: Absorbed drug undergoes metabolism in the liver

Indications for Loperamide1.DiarrhoeaInteractions for LoperamideN/ATypical Dosage for LoperamideAdult (Oral): Initial dose: 4 mg followed by 2 mg after each loose motion. Maintenance dose: 6 - 8 mg daily
Maximum dose: 16 mg / day
Children:
2 - 5 years: 3 mg / day in 3 divided doses
6 - 8 years: 4 mg / day in 2 divided doses
8 - 12 years: 6 mg / day in 3 divided doses
Schedule of LoperamideHStorage Requirements for LoperamideStore the drug at 15 - 30 degree C in a tightly closed container. Protect from heat and light.Effects of Missed Dosage of LoperamideDo not apply if it is taken as to control diarrhoea.
If it is taken as a regular schedule then take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of LoperamideGive supportive measures and symptomatic treatment. Drug can be removed from the body by gastric lavage. Absorption of the drug can be reduced by administration of activated charcoal. Respiratory depression can be treated with Naloxone. Furazolidone

Interactions for FurazolidoneAlcohol: Disulfiram like reaction-facial flushing,light headedness, weakness, lacrimation.
Anorexiants: Increased pressor response of anorexiants due to MAO inhibition.
Levodopa: Both efficacy and adverse effects of levodopa increased especially hypertensive crisis. Effect lasts for several weeks after stopping furazolidone.
Sympathomimetics (indirect & mixed): Increased pressor sensitivity to these agents due to MAO inhibition.
TCAs: Hypertension, hyperpyrexia, seizures, tachycardia, acute psychosis.
Hypnotics & Sedatives: Dose of hypnotics and sedatives should be reduced.
Hypoglycaemics: Potentiates them.
Food: Hypertensive crisis with tyramine containing foods.
Typical Dosage for FurazolidoneAdults: 100mg 6 hourly 2 to 7 days depending up on the severity of infections
Children: 1.25 mg 6 hourly for 2 to 5 days & up to 10days for Giardiasis
