A proton pump inhibitor, Benzimidazole derivative, Anti ulcer.
Mechanism of Action of Lansoprazole
Lansoprazole is a proton pump inhibitor. It is a prodrug. After administration it diffuses in to the parietal cell of the stomach and accumulates in the secretory canaliculi. In the acidic medium Lansoprazole is converted to sulfenamide. This sulfenamide covalently interacts with sulfhydryl (SH) group in the proton pump
(H+ K+ATPase) and inhibits the exchange of extracellular K+ for intracellular H+ ion. Lansoprazole irreversibly inhibits proton pumps activity and decreases gastric acid secretion.

Interactions for Lansoprazole
Antacids: Bio-availability of lansoprazole reduced.
Oral contraceptives, Phenytoin, Theophylline and Warfarin: Caution should be exercised while using these drugs which are metabolised by the liver since lansoprazole is a weak inducer of P450 cytochrome.
Typical Dosage for Lansoprazole
Adult: 15 - 30 mg / day to be taken before meals.
Duodenal ulcer: 15 mg / day before meals for 1 month.
Gastric ulcer: 30 mg / day for 2 months.
Zollinger- Ellison syndrome: 60 mg / day increase the dose to 180 mg/ day in 2 divided doses if needed.
Gastroesophageal reflux disease: 15 mg / day for 2 months.
Oesophagitis: 30 mg / day before meals for 2 months continue the treatment for 2 more months if needed.
Acid related dyspepsia: 15 - 30 mg / day before breakfast for 1 month.
NSAID associated ulceration: 15 - 30 mg / day for 1 - 2 months.

Children: not recommended

Mechanism of Action of Domperidone
Domperidone is a potent dopamine receptor antagonist. It acts centrally and blocks the Dopamine receptor in the Chemoreceptor trigger zone and produces Antiemetic effect.
Domperidone acts peripherally in the gastrointestinal system and increases oesophageal peristalsis, oesophageal sphincter pressure and gastric motility. These all facilitates gastric emptying. Domperidone is used in migraine to relieve nausea and vomiting.
Anaesthetic adjuncts: It is used preoperatively in order to reduce the post operative vomiting.

Digoxin: Oral dose of digoxin needs to be reduced.
Aspirin, Paracetamol & Oral Diazepam: Increases absorption of these agents.
Anticholinergic agents: Antagonize the effects of domperidone.
Phenothiazines: CNS depression enhanced by domperidone.
Antimuscarinic agents and opioids: Antagonize GI effects of domperidone.
Suxamethonium: Neuromuscular blockade enhanced by domperidone.
Typical Dosage for Domperidone
Adults: 30 - 40 mg / day in 3 - 4 divided doses 30 minutes before meals and at bed time if required. After 2 weeks dose can be increased to 60 - 80 mg / day if needed.
Migraine: 20 mg by mouth may be taken up to every 4 hours with Paracetamol, as required up to a maximum of 4 doses in 24 hours.
Children: 300 mcg / kg body weight 3 - 4 times daily 30 minutes before meals and at bed time if required
