A short acting Beta2-Adrenergic Agonist, Phenethylamine derivative, Bronchodilator(Antiasthma).
Mechanism of Action of Salbutamol (Inhalers)
Salbutamol is a short acting ?2 receptor agonist. It directly relaxes the airway smooth muscles and produces bronchodilation. Stimulation of ?2 receptors activates Gs adenylyl-cyclase -cyclic AMP path way and produces reduction in smooth muscle tone. ?2 receptor agonists also increases the conductance of large Ca2+-sensitive K+ channels in airway smooth muscles and leads to membrane hyperpolarisation and relaxation of smooth muscles.
?2 receptor agonists also suppress the release of Leukotrienes and histamine from the mast cells in the lung tissue and enhance mucociliary function and decrease micro vascular permeability and also inhibits Phospholipase A2 which produces inhibition of prostaglandins production. Thus Salbutamol can inhibit the broncho-constriction produced by inflammatory mediators.

Adult: Salbutamol 90mcg/metered spray
Relief of Bronchospasm in patients with reversible obstructive airway disease: 1 - 2 inhalations every 6 - 8 hours
Prevention of exercise-induced Bronchospasm: 200mcg 15 - 30 minutes before exertion
Chronic therapy:Up to 200mcg four times daily
Solutions for Nebulisation: 2 .5 mg 3 - 4 times daily by using nebulisers
Capsules for inhalation: 200mcg inhaled every 6-8 hours using rotahaler device.

Children: 100mcg, the dose may be increased to 200mcg if required.
Prevention of exercise-induced Bronchospasm: 100mcg before exertion, the dose may be increased to 200mcg if required.

Mechanism of Action of Ipratropium Bromide
Ipratropium bromide selectively acts on bronchial muscle without altering volume or consistency of respiratory secretions. It inhibits vagally mediated reflexes by antagonizing the actions of acetylcholine. It prevents the acetylcholine mediated increase in intracellular cyclic GMP concentration that results from interaction of acetylcholine with muscarinic receptor on bronchial smooth muscle. It does not depress the muco-ciliary clearance by bronchial epithelium. It blocks the cholinergic constrictor tone and acts primarily in the larger airways.
