Mechanism of Action of Sultamicillin
Sultamicillin is an oral form of the antibiotic combination ampicillin/sulbactam. It contains esterified ampicillin and sulbactam.
Sulbactam is an irreversible inhibitor of most important beta - lactamases, that occur in penicillin- resistant organisms.
Ampicillin is the bactericidal component of the combination, as it inhibits the biosynthesis of cell wall mucopeptide of many Gm +ve, Gm- ve aerobic and anaerobic bacteria.

Indications for Sultamicillin
1.Upper respiratory tract infections including sinusitis, otitis media and tonsillitis.
2.Lower respiratory tract infections including bacterial pneumonias and bronchitis.
3.Urinary tract infections and Pyelonephritis.
4.Skin and soft tissue infections and gonococcal infections.

Sultamicillin injection is also used for intra-abdominal infections including peritonitis, cholecystitis, endometritis, pelvic Cellulitis,bacterial septicemia, and also pre - operatively to reduce the incidence of postoperative wound infections.
Interactions for Sultamicillin
1.Concurrent use increases risk of bleeding with warfarin and methotrexate toxicity
2.Decreases efficacy of oestrgen-containing oral contraceptives.
3.Excretion of ampicillin is reduced when used with probencid.



Typical Dosage for Sultamicillin
Otitis media, Respiratory tract infections, Urinary tract infections

Adult: 375-750 mg bid.
Child: <30 kg: 25-50 mg/kg/day in 2 divided doses. ?30 kg: same as adult dose.

Uncomplicated gonorrhoea
Adult: 2.25 g as single dose, together with 1 g probenecid.
