Methyl Prednisolone
The drug exerts anti-inflammatory and immunosuppressant actions as follows: - 1) Induce lipocortins in macrophages, endothelium, and fibroblasts which inhibits phospholipase A2 and thus decreases the production of Prostaglandins, leukotriens (LT), and platelet activating factor, 2) Causes negative regulation of genes for cytokines in macrophages, endothelial cells and lymphocytes and thus decreases the production of interleukins (IL-1, IL-2, IL-3, IL-6), TNF-alpha, GM-CSF (granulocyte macrophage colony stimulating factor), Gama interferon and suppresses fibroblast proliferation and T-lymphocyte functions and interferes chemo taxis. 3) Decreases the production of acute phase reactants from macrophages and endothelial cells and interferes complement function. 4) Decreases the production of ELAM-1(Endothelial leukocyte adhesion molecule-1) and ICAM-1(intracellular adhesion molecule-1) in endothelial cells. 5) Inhibit IgE mediated histamine and LT-C4 release from basophiles and the effects of antigen-antibody reaction is not mediated 6) Reduces the production of collagenase and stromolysin and thus prevents tissue destruction

Vertigo: Initial dose: 40 - 80 mg / day for 4 - 5 days, followed by 20 mg / day fro 4 days.
Multiple sclerosis:200mg/day for one week; followed by 80mg every other day for one month.
Inflammation: 2 to 60mg/day in four divided doses depending up on the severity of the condition and patient`s response.
Allergic condition, Asthma, and Dermatological conditions: 40 to 120mg/day depending up on the severity of the condition and patient`s response.
Intralesional injection: 4 to 80mg depending up on the size of the lesion and severity of the condition.
I.V. or I.M.:
Allergic conditions, Ulcerative colitis, Crohn`s disease, Stevens-Johnson syndrome, Multiple sclerosis, Aspiration of gastric content, Transplantation, Cerebral oedema associated with tumor:
Starts with 10 to 500mg I.M. or slow I.V. infusion over half an hour; depending up on the severity of the condition. In transplant patients up to 1g/day, In multiple sclerosis 1g/day up to 3 days.
Oral
Children: 0.5mg/kg/day to 30mg/kg/day depending up on the disease type and severity of the condition.
Transplantation: 10 to 20mg/kg/day for 3days.
Asthma: 1 to 4mg/kg/day for 1 to 3 days.
Vertigo : 2 to 4mg/kg/day in 4 to 6 divided doses.
